Abstract

Description

Background: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease that causes muscles to waste and gives the person a gradual weakening of the body. Among other things, it leads to difficulties in moving, eating, and speaking. The disease is incurable, but slowing medication and symptom-relieving treatment are offered. Multidisciplinary team care is important for the person's care. It is also recommended to connect palliative care. Swallowing difficulties affect most people with ALS to varying degrees, which in the long run can lead to malnutrition and aspiration pneumonia. To avoid malnutrition and to prolong life, a gastrostomy is offered. Aim: The aim is to highlight, through a literature review, how people with ALS, their relatives and healthcare professionals experience decision-making around gastrostomy. Method: Systematic literature review with thematic analysis Results: The result is based on six articles. Three themes with nine subthemes emerged. Quality of life, acceptance, control, and information influenced the decision-making process of people with ALS and their relatives around gastrostomy. An unclear progression of the disease led to a delay in decisions, while an increase in symptoms meant that decisions were made when one considered himself to have no choice. There was a desire for healthcare professionals to provide clear information regarding gastrostomy and they preferred to receive it from the multidisciplinary team. Healthcare staff found it difficult regarding at what point in the disease's progression the information about the gastrostomy should be given. Conclusion: The literature study shows that quality of life was an individual experience that influenced decision-making around a gastrostomy. A palliative approach to the care of people with ALS and their relatives is a way of working to provide person-centred care. It can be used by healthcare professionals to, if possible, strengthen experiences of quality of life and control in people with ALS and their relatives in their decision-making around a gastrostomy. Teamwork was important regarding the need for information. A well-functioning team made people with ALS and their relatives experience greater satisfaction with their decision-making regarding a gastrostomy.
