Förutsättningar för jämlik personcentrerad palliativ vård

Abstract

Since the 1970s, several governmental investigations have provided thoughtful reports and suggestions on palliative and end-of-life care. In Sweden; however, the first national policy for palliative came into force in 2013, in tandem with the creation of the National Care Programme for Palliative Care. However, health professionals, patients, family carers and research results still point to unequal prerequisites for palliative care. This project was developed around the hypothesis that the introduction of the first palliative care policy should have contributed to increased implementation of palliative care. We assume this is reflected in an increasing proportion of the population who have received palliative care, with the opportunity to be cared for and die in places they prefer. Since Sweden lacks data on the preferred place of care at end of life, we assume that an increasing proportion of the population has died at home. Results from three research lines are presented herein: Swedish population preferences for place of care at end of life and death, population trends for place of death, and perspectives on national policy and governance of palliative care. The majority of the population prefer to die at home. The national policy on palliative care is weak and unclear, and the use of care at the end of life is unequal and hospital-dominated. Based on the overall results, proposals for governance reforms are suggested, including recognising palliative care as a priority public health issue, and making national palliative care policy binding.

Sedan 1970-talet har flera statliga utredningar resulterat i genomtänkta rapporter och förslag om palliativ vård och vård i livets slutskede. I Sverige trädde dock den första nationella policyn för palliativ vård i kraft först 2013. Samtidigt introducerades det nationella vårdprogrammet för palliativ vård. Erfarenheter från hälso- och sjukvårdspersonal, patienter och närstående tillsammans med forskningsresultat pekar dock fortfarande på ojämlika förutsättningar för palliativ vård. Detta projekt utvecklades utifrån hypotesen att införandet av den första nationella policyn för palliativ vård har bidragit till en ökad implementering av palliativ vård. Vi antar att detta återspeglas i en ökande andel av befolkningen som har fått palliativ vård, med möjlighet att vårdas och dö på platser som de föredrar. Eftersom Sverige saknar data om önskad vårdplats i livets slutskede antar vi att en ökande andel av befolkningen har dött i hemmet. Resultat från tre forskningslinjer presenteras: var den svenska befolkningen föredrar att vårdas i livets slutskede och dö, trender för plats för död i befolkningen samt perspektiv på nationell policy och styrning av palliativ vård. Majoriteten av befolkningen föredrar att dö hemma. Nationell policy för palliativ vård är svag och oklar. Nyttjandet av vård i livets slutskede är ojämlik och domineras av sjukhusvård. Baserat på de övergripande resultaten ges förslag till reformer för styrning av palliativ vård. Dessa inkluderar att palliativ vård ska erkännas som en prioriterad folkhälsofråga och att nationell policy för palliativ vård ska göras bindande.
