Påverkan på HbA1c av precision nutrition baserad på CGM hos vuxna med prediabetes/diabetes typ 2 - En systematisk översiktsartikel

Abstract

Syfte: Denna systematiska översikt syftar till att undersöka påverkan av precision nutrition (PN) baserad på kontinuerlig glukosmätning (CGM) på glykosylerat hemoglobin (HbA1c) hos individer med diabetes mellitus typ 2/prediabetes. Metod: Litteratursökningen genomfördes i slutet av januari 2025 i databaserna PubMed och Scopus. Den inkluderade populationen bestod av vuxna individer med/utan insulinbehandlad diabetes typ-2. Interventionen var kostbehandling baserad på CGM-mätningar med/utan andra biomarkörer. Utfallsmåttet var HbA1c. Studier exkluderades om de inte var skrivna på engelska eller svenska, hade mindre än tio deltagare, omfattade populationer med graviditetsdiabetes eller var djurstudier. Sökstrategin använde tre block, där block 1 innefattade PN, block 2 diabetes och prediabetes, och block 3 RCT-studier. Artiklarna granskades med SBU:s granskningsmall för att bedöma risk för bias. Tillförlitlighet bedömdes enligt WCRF/NNR för utfallsmåttet HbA1c. Resultat: Totalt identifierades 103 träffar. Efter dubblettkontroll med referenshanteringsverktiget Zotero kvarstod 71 artiklar. En gemensam screening genomfördes utifrån fördefinierade inklusions- och exklusionskriterier, vilket resulterade i att 6 artiklar valdes ut för fulltextgranskning. Två ytterligare artiklar identifierades under fulltextgranskningen och inkluderades efter screening av titel och abstract. Slutligen lästes åtta artiklar totalt och tre av dessa inkluderades i den slutliga analysen. Två artiklar bedömdes ha hög risk för bias och en artikel bedömdes ha måttlig risk för bias. Dessa tre studier utgjorde en studiepopulation på 765 personer. Interventionen pågick i sex till tolv månader. Statistisk signifikant effekt sågs för utfallsmåttet HbA1c i två studier. Det sammanvägda resultatets tillförlitlighet evidensgraderas som begränsad – otillräcklig (otillräcklig evidens) för att HbA1c påverkas av PN baserad på CGM-data hos personer med diabetes typ 2/prediabetes. Slutsats: Det finns ett begränsat antal RCT-studier där CGM användes inom PN. Fler studier av bättre kvalitet behövs, därmed kan inga slutsatser dras.

Aim: This systematic review aims to examine the effect of precision nutrition (PN) based on continuous glucose monitoring (CGM) on glycated hemoglobin (HbA1c) in individuals with type 2 diabetes/prediabetes. Methods: The literature search was conducted 31 January 2025 in the PubMed and Scopus databases, focusing on randomized controlled trials (RCTs). The included population consisted of adult individuals, with or without insulin dependent diabetes type 2. The intervention involved dietary management based on CGM measurements, with or without additional biomarkers. The primary outcome measure was HbA1c. Studies were excluded if they were not published in English or Swedish, had fewer than ten participants, involved populations with gestational diabetes, or were animal studies. The search strategy utilized three blocks: block 1 was about PN, block 2 was about diabetes/prediabetes, and block 3 was about RCTs. Articles were assessed for risk of bias using SBU’s critical appraisal tool, and the reliability of the results was evaluated according to WCRF/NNR for the outcome measure HbA1c. Results: A total of 103 articles were identified. After duplicate removal using the reference management tool Zotero, 71 articles remained. A joint screening was conducted based on predefined inclusion and exclusion criteria, resulting in six articles being selected for full-text review. Two additional articles were identified during the full-text review phase and included after screening of the title and abstract. In total, eight articles were fully reviewed, and three of these were included in the final analysis. Two articles were assessed as having high risk of bias, while one article was assessed as having some concerns. These three studies comprised a total study population of 765 participants. The intervention duration was six to twelve months. Statistically significant effect was observed for the outcome measure HbA1c in two studies. The reliability of the overall results is graded as limited – no conclusion (insufficient) regarding the effect of PN based on CGM-data on HbA1c in individuals with type 2 diabetes or prediabetes. Conclusion: There are a limited number of RCTs where CGM was used in PN. More studies of better quality are needed; therefore no conclusions can be drawn.
