Effekten av vitamin D på astma - En systematisk översiktsartikel om vitamin D-supplementering hos vuxna med diagnostiserad icke-allergisk astma

Abstract

Syfte: Syftet med denna systematiska översiktsartikel var att undersöka det vetenskapliga underlaget avseende effekten av vitamin D-supplement på ickeallergisk astma hos vuxna. Metod: Litteratursökningen genomfördes i två databaser, PubMed och Scopus. Sökningen genomfördes med hjälp av tre sökblock. Blocken specificerades efter innehåll; astma, vitamin D och randomiserade kontrollerade studier (RCT). Studierna som inkluderades i denna systematiska översiktsartikel var gjorda på vuxna personer med icke-allergisk astma och utan samsjuklighet. Interventionen var vitamin D-supplement och kontrollen var placebo. De tre valda utfallsmåtten var FEV1, FVC och FEV1/FVC och mättes med spirometri. Studier exkluderades om de inkluderade gravida, parallell behandling med andra preparat och om de var gjorda på djur. Studier som mötte givna kriterier inkluderades och bedömdes för risk för bias med hjälp av mallen ”Bedömning av risk för bias i randomiserade kontrollerade studier (RCT)”, utformad av Statens beredning för medicinsk och social utvärdering (SBU). Tillförlitligheten avseende de inkluderade studiernas resultat bedömdes därefter med hjälp av mallen ”Mall för evidensgradering”, utformad Göteborgs Universitet, Sahlgrenska Akademin, avdelning invärtes medicin och klinisk nutrition. Resultat: Litteratursökningen resulterade i 227 unika träffar efter hantering av dubbletter. Av dessa lästes 14 artiklar i fulltext varav fyra uppfyllde samtliga kriterier. Av tre givna utfallsmått var det endast FVC som uppvisade samstämmiga resultat, dock utan signifikans. För utfallsmåtten FEV1 och FEV1/FVC varierade antalet signifikanta resultat mellan de olika studierna. Slutsats: Resultaten från de ingående studierna visar på att det saknas samstämmighet för FEV1 och FEV1/FVC. Avseende utfallsmåttet FVC är resultaten samstämmiga men inte statistiskt signifikanta. Det går inte att med säkerhet säga att vitamin D-supplement har effekt på icke-allergisk astma hos vuxna. Graden av evidens bedöms därmed vara Otillräcklig – begränsad. Ytterligare forskning kring ämnet är önskvärd.

Aim: The aim of this systematic review article was to investigate the scientific basis regarding the effect of vitamin D supplementation on non-allergic asthma in adults. Methods: The literature search was conducted in two databases, PubMed and Scopus. The search was performed using three search blocks. The blocks were specified by topics; asthma, vitamin D and randomized controlled trials (RCT). The studies included in this systematic review article were performed in adults with non-allergic asthma and without any other comorbidity. The intervention was vitamin D- supplementation and the control received placebo. The three chosen outcomes were FEV1, FVC and FEV1/FVC and were measured by spirometry. Studies were excluded if they included pregnancy, parallel treatment with other agents and if they were conducted in animals. Studies that met the specified criteria were included and was assessed for risk of bias using the template “Bedömning av risk för bias i randomiserade kontrollerade studier (RCT)”, developed by Statens beredning för medicinsk och social utvärdering (SBU). The reliability of the results in the included studies was assessed using the template “Mall för evidensgradering”, developed by the University of Gothenburg, Sahlgrenska Academy, Department of Internal Medicine and Clinical Nutrition. Results: The literature search resulted in 227 unique hits after handling duplicates. Of these, 14 were read in full text of which four of these met all the criteria. Of the three specified outcomes, only FVC showed consistent results, although without statistical significance. For the outcomes FEV1 and FEV1/FVC the number of statistic significant results varied across the different studies. Conclusion: The results from the included studies show that there is a lack of consistency for FEV1 and FEV1/FVC. Regarding the outcome measure FVC,the results are consistent but not statistically significant. It is not possible to say with certainty that vitamin D supplementation has an effect on non-allergic asthma in adults. The level of evidence is therefore assessed to be Insufficient – limited. Further research on the topic is desirable.
