BLÖDNINGENS OSYNLIGA BERÄTTELSE - Faktorer som påverkar upplevelsen av en postpartumblödning

Abstract

Background: Postpartum hemorrhage (PPH) is a leading cause of maternal mortality globally, particularly in low-income countries, and its incidence has also increased in high income countries. PPH can have serious medical and psychological consequences, including impaired bonding and delayed initiation of breastfeeding. Preventive measures, early detection, and appropriate management are crucial, as is the midwife’s role in providing support, information, and promoting family-centered care. Family-centered care and respect for sexual and reproductive rights contribute to safety, participation, and improved recovery for both the woman and her partner. Aim: The aim of this study is to explore the factors that influence women’s and their partners’ experiences of PPH. By including the perspectives of both women and their partners the study seeks to contribute to a deeper understanding of their experiences and the challenges they face. Method: A systematic literature review with an integrative design was conducted by Whittemore and Knafl. 14 scientific articles have been included in the results, the literature search was carried out in the databases PubMed and CINAHL. Results: The results of the literature review identified two main categories, Healthcare professionals’ communication and approach, and Psychological and physical challenges postpartum. The findings indicate that women’s and partners’ experiences of information before, during and after a postpartum hemorrhage, as well as the healthcare professionals’ calmness and presence, play a significant role in the sense of safety. Clear communication reduced fear, whereas insufficient information increased feelings of loss of control. Acute medical interventions, separation from the infant, and maternal physical weakness affected early caregiving and contributed to feelings of anxiety, guilt, and inadequacy for both women and their partners. Conclusion: PPH is a frightening experience for both women and their partners. Healthcare professionals’ communication, calmness, and presence are crucial for creating a sense of safety, while insufficient information and limited involvement can lead to fear and psychological distress. The midwife plays a central role in supporting recovery and promoting family-centered care.
