Abstract

Bakgrund: Antalet äldre multisjuka ökar vilket ställer kompetenskrav på personal som vårdar dessa personer. Många har insatser av kommunal primärvård vilket innebär att samverkan mellan regional och kommunal primärvård är central för god, säker och personcentrerad vård, vilket uppnås bl.a. genom upprättande av individuell vårdplan. Det finns tidigare inte beskrivet om sjuksköterskors erfarenheter av samverkan mellan regional och kommunal primärvård och om en individuell vårdplan för diabetes kan underlätta vid samverkan och vårdandet av patienterna. Syfte: Syftet är att undersöka erfarenheter av samverkan mellan regional och kommunal primärvård avseende vården av personer med diabetes mellitus typ 2, ur ett sjuksköterskeperspektiv Metod: Kvalitativ metod med semistrukturerat frågeunderlag i fokusgrupper som analyserats med tematisk analys. Två fokusgrupper med sex sjuksköterskor i varje grupp. Resultat: I resultatet användes de tre forskningsfrågorna som övergripande teman med vardera två underteman. Förutsättningar för samverkande och vårdande med underteman Gemensamt ansvarstagande och Kontinuitet i arbetsrelationen. Här framkom att det var viktigt med varaktiga professionella relationer och att sjuksköterskorna kring patienten känner sig inkluderade i vårdandet. Hinder för samverkande och vårdande med underteman Oklara rutiner och riktlinjer och Otillräcklig erfarenhet och kompetens. Bristande rutiner och kommunikation kring patienterna och hög personalomsättning var hinder som identifierades. Individuell vårdplan som stöd med underteman Bidra med trygghet vid bedömning och Ge en helhetsbild av personen med diabetes. Det framkom att en individuell vårdplan skulle kunna skapa förutsättningar för trygga bedömningar av patienterna, ge en övergripande och konkret plan, helhetssyn på patienten och mer personcentrerad vård. Slutsats: Samverkan mellan regional och kommunal primärvård krävs för att vården av personer med diabetes typ 2 ska vara säker, av god kvalitet och personcentrerad. Ett införande av individuella vårdplaner kan vara ett stöd i denna samverkan och i vårdandet av patienterna.

Background: The number of elderly people with multiple illnesses is increasing, which places competence demands on staff who care for these people. Many have interventions of municipal primary care, which means that cooperation between regional and municipal primary care is central to good, safe and person-centered care, which is achieved among other things through the establishment of an individual care plan. However, it remains unclear how nurses perceive this cooperation and whether the implementation of individual care plans for diabetes management can facilitate cooperation and patient care. Aim: The aim is to explore experiences of cooperation between regional and municipal primary healthcare regarding the care of people with diabetes mellitus type 2. Method: A qualitative methodology using focus groups for data collection. The focus group transcripts were analyzed with thematic analysis. Results: Two focus groups with six nurses in each group. In the result, the three research questions were used as overarching themes and six associated subthemes were located. Prerequisites for cooperation and nursing with the subthemes Mutually responsibility and Continuity in the employee relationship. It appeared that it was important to have lasting professional relationships and the importance that the nurses around the patient all felt included. Barriers for cooperation and nursing with the subthemes Unclear routines and guidelines and Inadequate experience and competence. Lack of routines and communication and high employee turnover were barriers that were identified. Individual care plan as support with sub-themes Contribute with safety during medical assessment and Contribute with a holistic focus of the person with diabetes. Here it emerged that an individual care plan could set the conditions for safe medical assessments of patients, provide a comprehensive and concrete plan, a holistic view of the patient and more person-centered care. Conclusion: Cooperation between regional and municipal primary care is required for the care of people with type 2 diabetes to be safe, of good quality and person-centred. The introduction of individual care plans can be a support in this cooperation and in the care of the patients.
