Abstract

Bakgrund: Operationssalen är en komplex och resursintensiv miljö där effektiva patientflöden och optimalt resursutnyttjande är centrala för patientsäkerhet och en hållbar hälso- och sjukvård. Förlängd anestesikontrollerad tid har associerats med ökad risk för perioperativa komplikationer, fördröjda patientflöden och ökade vårdkostnader. Tidigare forskning har huvudsakligen fokuserat på tekniska faktorer som påverkar anestesikontrollerad tid, medan betydelsen av teamrelaterade och organisatoriska faktorer som kan relateras till operationsteamets icke-tekniska färdigheter är mindre utforskad. Syfte: Att undersöka samband mellan teamrelaterade och organisatoriska faktorer i operationssalen och anestesikontrollerad tid. Metod: En systematisk litteraturöversikt med kvantitativ ansats genomfördes. Sökningarna utfördes i databaserna PubMed och Scopus. Kvantitativa peer review-granskade artiklar publicerade på engelska under de senaste tio åren inkluderades. Tio kohortstudier valdes ut och kvalitetsgranskades med hjälp av CASP:s granskningsmall för kohortstudier. Data analyserades genom tematisk sammanställning. Resultat: Tre teman identifierades: förtrogenhet i teamet, standardiserade och teambaserade arbetssätt samt samband mellan specialistnärvaro och operationsflöde. Högre grad av förtrogenhet och kontinuitet i teamet var associerat med kortare extubations- och omsättningstider, vilka kan betraktas som närliggande tidsmått kopplat till anestesikontrollerad tid. Standardiserad kommunikation och teaminterventioner visade minskade perioperativa tidsintervall med nära koppling till anestesikontrollerad tid, särskilt avseende tiden från ankomst till operationssal till kirurgisk incision samt från sårförslutning till att patienten lämnade operationssalen. Specialistnärvaro under kritiska perioperativa moment var associerat med kortare tid från ankomst till operationssal till anestesiinduktion samt från ankomst till operationssal till kirurgisk incision. Slutsats: Teamrelaterade och organisatoriska faktorer i operationssalen, såsom förtrogenhet, strukturerade arbetssätt och specialistnärvaro, förefaller vara associerade med anestesikontrollerad tid samt närliggande perioperativa tidsmått. Dessa faktorer kan teoretiskt relateras till operationsteamets icke-tekniska färdigheter, men utgör inte direkta mått på sådana färdigheter. Resultaten talar för att teamrelaterade och organisatoriska faktorer, som kan förstås som indirekta indikatorer på icke-tekniska färdigheter, kan ha betydelse för anestesikontrollerad tid och perioperativ effektivitet. Evidensen är dock indirekt, då de inkluderade studierna inte mätte icke-tekniska färdigheter direkt utan använde närliggande teamrelaterade och organisatoriska faktorer som indikatorer.

Description

Background: The operating room is a complex and resource-intensive environment where efficient patient flow and optimal use of resources are essential for patient safety and healthcare sustainability. Prolonged anesthesia-controlled time has been associated with increased risk of perioperative complications, delayed patient flow, and increased healthcare costs. Previous research has primarily focused on technical factors affecting anesthesia-controlled time, whereas the importance of team-related and organizational factors that may be associated with the operating team’s non-technical skills has been less explored. Aim: To examine the relationship between team-related and organizational factors in the operating room and anesthesia-controlled time. Method: A systematic literature review with a quantitative approach was conducted. Searches were performed in the databases PubMed and Scopus. Quantitative peer-reviewed studies in English published within the last ten years were included. Ten cohort studies were selected and critically appraised using the CASP checklist for cohort studies. Data was analyzed through a thematic summary of findings. Results: Three themes were identified: team familiarity, standardized and team-based working methods, and the association between specialist presence and operating room flow. Higher levels of team familiarity and continuity were associated with shorter extubation and turnover times, which can be considered proximal time measures linked to anesthesia-controlled time. Standardized communication and teamwork interventions demonstrated reductions in perioperative time intervals closely related to anesthesia-controlled time, particularly in the time from operating room arrival to surgical incision and from wound closure to patient exit from the operating room. Specialist presence during critical perioperative stages was associated with shorter time from operating room arrival to anesthesia induction and from operating room arrival to surgical incision. Conclusion: Team-related and organizational factors in the operating room, such as familiarity, structured work practices, and specialist presence, appear to be associated with anesthesia-controlled time and related perioperative time metrics. These factors can theoretically be related to the surgical team’s non-technical skills, but they do not constitute direct measures of such skills. The results suggest that team-related and organizational factors, which may be understood as indirect indicators of non-technical skills, may be important for anesthesia-controlled time and perioperative efficiency. The evidence is, however, indirect, as the included studies did not measure non-technical skills directly but used related team-based and organizational factors as indicators.
