Mediterranean diet intervention can reduce degree of depression – a systematic review
Abstract
Syfte:
Syftet med denna systematiska översikt var att undersöka medelhavskostens påverkan på grad av depression hos vuxna personer med depression.
Metod:
Sökningar utfördes i databaserna PubMed och Scopus under mars 2023. Inklusionskriterier var randomiserade kontrollerade studier, vuxna ≥18 år med diagnostiserad depression enligt självskattningsformulär eller medicinskt utlåtande. Utfallsmåttet var grad av depressionssymtom mätt med skalorna MADRS (Montgomery-Åsberg Depression Rating Scale), BDI-II (Beck Depression Inventory II) eller DASS-21 (Depression Anxiety Stress Scale).
Artiklar som inkluderat fler interventioner i sina studier än kostintervention eller artiklar som ej var skrivna på engelska exkluderades.
De studier som mötte kriterierna inkluderades och kvalitetsgranskades enligt SBU:s (Statens beredning för medicinsk och social utvärdering) granskningsmall ”Bedömning av randomiserade studier – effekt av att tilldelas intervention”. Den sammanvägda tillförlitligheten för studiernas resultat bedömdes enligt GRADE (Grading of Recommendations Assessment Development and Evaluation).
Resultat:
Litteratursökningen resulterade i 169 unika träffar. Artiklar som lästes i fulltext var 18 st varav tre artiklar inkluderades i denna systematiska översikt. Risken för bias var måttlig i två utav studierna och hög i tredje studien.
Resultatet var samstämmigt mellan studierna som visade att medelhavskost kan påverka graden av depressionssymtom hos deprimerade vuxna ≥18 år. Minskning i grad av depressionssymptom i interventionen jämfört med kontrollen resulterade enligt följande: HELFIMED –43,6 %, SMILES –62,8 % och AMMEND –69,9 %.
Slutsats:
Medelhavskost kan minska graden av depressionssymtom. Det vetenskapliga underlaget visade Måttlig tillförlitlighet (+++).

Aim:
The purpose of this systematic review was to investigate the impact of the Mediterranean diet on the degree of depression in adults with depression.

The intervention was a Mediterranean diet and the control group received social treatment in means of social discussions and group sessions. Inclusion criteria were randomized controlled trials, adults ≥18 years with diagnosed depression according to self-report questionnaires or medical opinion. The outcome measure was degree of depression symptoms measured with the scales MADRS (Montgomery-Åsberg Depression Rating Scale), BDI-II (Beck Depression Inventory II) or DASS-21 (Depression Anxiety Stress Scale).
Articles that included more interventions in their studies than dietary intervention or articles that were not written in English were excluded.
The studies that met the criteria were included and quality reviewed according to SBU´s (Statens beredning för medicinsk och social utvärdering) template " Bedömning av randomiserade studier – effekt av att tilldelas intervention ". The quality of evidence regarding the results of the studies was assessed according to GRADE (Grading of Recommendations Assessment Development and Evaluation).
Results:
The literature search gave 169 results. Articles that were read in full text were 18 of which three articles were included in this systematic review. Two of the studies had moderate risk of bias and the third study had high risk. The results were consistent between the studies showing that a Mediterranean diet can affect the degree of depression symptoms in depressed adults ≥18 years.
Reduction in the degree of depression symptoms in the intervention compared to the control resulted as follows: HELFIMED –43.6%, SMILES –62.8% and AMMEND –69.9%.
Conclusion:
Mediterranean diet can reduce the degree of depression symptoms. The quality of evidence showed Moderate certainty (+++).
