Abstract
Bakgrund: Allt fler patienter idag är i behov av palliativ vård. Patienter med obotlig övre
gastrointestinal (ÖGI) cancer på Sahlgrenska Universitetssjukhuset erbjuds i dag vårdformen
bästa understödjande behandling (BSC). BSC är en palliativ behandling utan livsförlängande
åtgärder och som även ges på symtombasis. Med denna vårdform vilar ansvaret på patienten
att kontakta sjukvården när nya symtom uppstår eller tidigare symtom förvärras istället för en
systematisk uppföljande palliativ vård. Det behövs mer kunskap om hur dessa patienter
upplever BSC för att vidareutveckla vården för denna patientgrupp.
Syfte: Att undersöka hur patienter med obotlig ÖGI cancer upplever den palliativa vården de
får idag i form av BSC.
Metod: En kvalitativ intervjustudie med semistrukturerade intervjuer. Åtta patienter, fem
män och tre kvinnor mellan åldrarna 71-89 år med erfarenhet av BSC intervjuades. Data
analyserades med kvalitativ innehållsanalys enligt Graneheim och Lundman (2004).
Resultat: Resultatet utmynnade i två kategorier: Brister med BSC och Positiva aspekter med
BSC. De underliggande subkategorierna Otillräcklig information och kommunikation,
Oklarheter angående ansvarsfördelning mellan organisationen, patienter och anhöriga och
Otillräckligt stöd vid symtomhantering beskrev hinder som deltagarna upplevde som brister
med BSC. Subkategorierna God delaktighet för patient och närstående, Gott bemötande och
god tillgänglighet och En känsla av trygghet beskrev vilka förutsättningar som upplevdes
bidra till god BSC. Patienterna upplevde ibland informationen som otydlig vilket bidrog till
att utrymme lämnades för egen tolkning av informationen. De var nöjda med bemötandet från
vårdpersonalen och kände sig trygga med att kunna kontakta vården när de behövde.
Slutsats: BSC hade vissa brister enligt deltagarna, bland annat var informationen delvis
bristfällig och flera deltagare upplevde sig otillräckligt symtomlindrande. Därav behöver
informationen anpassas för varje patient och exempelvis ges genom återkommande samtal
med patienter och anhöriga. Det skulle kunna främja patientdelaktighet vilket är en viktig
aspekt inom personcentrerad palliativ vård.

Background: More patients than ever are in need of palliative care today. Patients with
untreatable upper gastrointestinal cancers at Sahlgrenska University Hospital are today
offered best supportive care (BSC). BSC is a form of palliative care focusing on nonlife
extending treatments and symptom management. As of now the responsibility is on the
patient to report escalating existing and new symptoms instead of a more systematic
palliative care. More knowledge is needed about how these patients experience BSC in order
to further develop the care for this group of patients.
Aim: To describe patients with untreatable upper gastrointestinal cancers’ experiences of the
palliative care they receive in the form of BSC.
Method: A qualitative interview study with semi-structured interviews. Eight patients, five
men and three women between the ages of 71-89 with experiences concerning BSC were
interviewed. The data were analyzed using qualitative content analysis according to
Graneheim and Lundman (2004).
Results: The result formed two categories: Shortcomings with BSC and Positive aspects with
BSC. The underlying subcategories Insufficient information and communication, Ambiguity
in the division of responsibilities between the organization, patients, and relatives and
Insufficient support with symptom management described the hindrances’ participants
experienced to conduct BSC. The subcategories Active involvement for patients and their
relatives, Good Approach and accessibility and A sense of security described the prerequisites
experienced by the participants to conduct BSC.
Conclusion: According to the participants, BSC had some shortcomings. Some of these
shortcomings being that the information was partially inadequate and that several participants
experienced insufficient symptom management. The information therefore needs to be
adapted for each patient and, for example, given through recurring conversations with
patients and relatives. That could promote patient participation which is an important aspect
of person-centered palliative care.
