Body image in people with eating disorders- the psychiatric nurse´s experiences
Abstract
Bakgrund: Ätstörningar är en samling av psykiatriska diagnoser som orsakar stort lidande
och ger allvarliga somatiska, psykiska och sociala följder. Kroppsuppfattningen hos personer
med ätstörning är ofta påverkad på ett negativt sätt. Det centrala för ätstörningar är att
personens tankar upptas av en negativ kroppsuppfattning eller störd kroppsuppfattning.
Problematiken kring den negativa kroppsuppfattningen skapar en oro och ångest samt verkar
vara en av de faktorer som tycks vara starkt kopplad till ätstörningssjukdom i olika stadier;
insjuknande, vidmakthållande samt återinsjuknande. Dessutom är relationen mellan
vårdpersonal och patienter ofta utmanande då den drabbade kan vara ambivalent inför
behandling samt kan ha svårigheter att engagera sig i sin behandling.
Syfte: Att beskriva hur specialistsjuksköterskor inom psykiatrisk vård arbetar med
kroppsuppfattning hos vuxna personer med ätstörningar.
Metod: Vi har utfört en kvalitativ tematisk analys, med induktiv ansats, av semistrukturerade
intervjuer med åtta specialistsjuksköterskor inom psykiatrisk vård. Deltagarna hade minst ett
års yrkeserfarenhet av att arbeta inom specialiserad ätstörningsvård.
Resultat: I resultatet framkom tre teman; (1) Tolkning av patientens kroppsuppfattning, (2)
Behandlande åtgärder och (3) Kompetens, samt flera subtema under varje tema. Flera av
deltagarna beskrev att de saknade verktyg för att arbeta med patientens kroppsuppfattning.
Utmaningar, svårigheter och behov i arbetet med kroppsuppfattningen beskrivs i resultatet.
Slutsats: Utifrån specialistsjuksköterskans kontext är arbetet med kroppsuppfattningen
mycket komplex. Det finns ett stort behov av mer utbildning och handledning för att känna
trygghet i bedömning och behandling. För att nå fram till patienten och kunna skapa en
terapeutisk allians behöver patienten känna sig trygg för att våga öppna upp sig och berätta
sin livshistoria. Ytterligare forskning både ur patientens och specialistsjuksköterskans
perspektiv behövs.

Background: Eating disorders are psychiatric diagnoses that cause suffering and serious
somatic, psychiatric, and social consequences. Persons with eating disorders commonly have
a negative body image. Central for eating disorders is that the person's thoughts are taken up
by a negative body image or a disturbed body image. The issues around the negative body
image creates concerns and anxiety and seem to be one of the core factors that are strongly
connected to the eating disorder in different stages; manifesting the illness, maintenance and
relapse. Besides, the relationship between healthcare staff and patients is often challenged
when the affected can be ambivalent to treatment and have difficulties involving in their
treatment.
Aim: To describe how psychiatric nurses within psychiatric nursing care works with body
image in patients with eating disorders.
Method: A qualitative content analysis with an inductive approach. The semi-structured
interviews were completed with eight mental health nurses within psychiatric healthcare with
at least one year of professional experience working within a specialized eating disorders unit.
Results: The result emerged in three themes; (1) Interpretation of the patient's body image,
(2) Treatments, and (3) Competence and several sub-themes. Half of the participants
described that they were lacking tools for working with patients' body image. Challenges,
difficulties, and the need to work with body image is described in the result.
Conclusion: From the psychiatric nurse context, working with body image is very complex
where the patient often has a disturbed body image. The participants experience a great need
for more education and tutorial to feel safe in assessment and treatment. To reach the patient
and to create a therapeutic alliance the patient needs to feel safe to dare to open up and talk
about their life story. Additional research is needed both from the patients and the psychiatric
nurse perspective.
