dc.description.abstractCurrent knowledge regarding disorders in work with hand-held vibrating machines : Medical aspects A critical review has been made of the literature with respect to the relationship between possible health risks and exposure to various factors in work with hand-held vibrating machines. The critical effects of interest in this context are disturbances in the blood circulation of the hand in the form of so-called white fingers, and disturbances of the nerve functions of the hand manifested as diffusely distributed neuropathy. Carpal tunnel syndrome and reduced muscle power in the hand cannot be regarded, on the basis of current knowledge, as critical effects of exposure to hand-arm vibration. Available information does not support the notion that disorders of bones and joints are specific to this exposure. Arthrosis of the wrist and elbow joints may be secondary to the added strain connected with the handling of percussive tools. The review has shown that there are no valid exposure-response-relationships permitting quantitative recommendations for a limitation of exposure. The goal should be to achieve a general reduction of exposure by preferring machines with lower vibration level, as determined according to the Swedish standard, SS-ISO 5349, together with technical measures in the workplace. Continued research is required to establish quantitative exposure-response-relationships that reflect the decisive risk factors connected with different machine types, work processes and work methods as well as with specific biodynamic and biomechanical factors. This research should be carried out through prospective studies. It is of value to collect non-frequency-weighted acceleration data for later analysis in various ways. Considering the difficulties of preventing vibration injury with the help of threshold limit values, medical surveillance may be valuable for the prevention of advanced vibration-induced disorders. The medical surveillance should comprise an examination at the start of a job and later examinations at intervals. This requires that the work organization allows the identification of suspected cases of vibration-induced disturbances and that those employees will be given the opportunity to consult a doctor experienced in these disorders. The precise form of such "medical criteria" cannot be stated in ordinances, since the need, as well as the adequate procedures, vary between occupational groups, work processes, and individual work methods.eng
dc.description.abstractEn kritisk litteraturgenomgång har gjorts med avseende på sambandet mellan eventuella hälsorisker och exponering för olika faktorer i arbete med handhållna vibrerande maskiner. De kritiska effekter som är aktuella i detta sammanhang är cirkulationsstörning i händerna i form av s.k. vita fingrar samt störningar i handens nervfunktioner i form av diffust utbredd neuropati. Karpaltunnelsyndrom och nedsatt muskelkraft kan på grundval av dagens kunskap inte anses vara kritiska effekter av exponering för hand-arm-vibrationer, och tillgänglig dokumentation ger inget stöd för specifika ben- och ledåkommor vid denna exponering. Artros i handled och armbågsled kan ha samband med den extra belastning som det innebär att hantera stötande handhållna maskiner. Litteraturgenomgången har visat, att det inte föreligger valida exponerings-respons-relationer som möjliggör kvantitativa rekommendationer om exponeringsbegränsning på grundval av nuvarande kunskap. Minskning av exponering bör eftersträvas genom att maskiner med lägre vibrationsnivå, mätt på grundval av den svenska standarden SS-ISO 5349, föredras liksom genom tekniska åtgärder på arbetsplatsen. Fortsatt forskning krävs för att fastställa kvantitativa exponering-respons-samband anpassade till de riskavgörande faktorer, som är förknippade med olika maskintyper, arbetsprocesser och arbetssätt samt med biodynamiska och biomekaniska faktorer. Sådan forskning bör bedrivas genom prospektiva studier. Det är av värde för fortsatt forskning att icke-frekvensvägda accelerationsdata insamlas för senare analys på olika sätt. Med tanke på svårigheten att utifrån exponeringsgränsvärden förebygga mera avancerade vibrationsskador kan i stället medicinsk övervakning vara av värde. Denna bör omfatta exempelvis anställningsundersökning och därefter intervallvisa undersökningar av vibrationsexponerade användas i preventivt syfte. Detta förutsätter att arbetsorganisationen är sådan att fall av misstänkt skadlig vibrationspåverkan fångas upp och att de anställda ges tillfälle till läkarkonsultation. Formerna för dessa "medicinska kriterier" kan inte fastslås i föreskrifter, eftersom behovet. liksom också de lämpliga procedurerna, varierar med yrkesgrupp, arbetsprocesser och individuellt arbetssätt.swe
